The following forms are designed to help us work together more easily...
New Client Packet We are currently requiring that all clients, whether new or already established with our practice, take the time to complete the entire New Client Packet, and bring those completed and signed copies with them to their first appointment beginning with our August re-opening. This packet includes updated Health History, HIPPA/Patient Privacy, General Informed Consent, Office Policies, and COVID-19 Informed Consent forms. If you do not bring the completed forms with you to your appointment, your massage will be rescheduled.
Additional Forms If the appointment is for a minor, someone who is pregnant, or if you are receiving a hot stone massage, please also print off and complete the applicable forms as listed below.
Thank you for printing and filling out your forms in advance, and bringing the completed copies with you to your appointment!
This is required for ALL clients. It must be printed off, completed in it's entirety, and brought in for your first appointment. If you do not have this completed and with you at your first appointment, your massage will be rescheduled.
This form must be filled out and signed by the parent or legal guardian of a minor (anyone under the age of 18) before that minor can be treated in our office.
Please fill out and sign this additional form if you are pregnant and will be receiving massage in our clinic.
Hot Stone Massage Release Form
If your appointment is for Hot Stone Massage, you will need to complete this form before your appointment.
